What should I do first if my chainsaw chain keeps coming off?

Tighten the chain. It’s almost always because the chain tension is too low — it should be snug against the bottom of the bar rail with no sag but still able to spin freely by hand. If tensioning doesn’t fix it, check the bar rails for wear or the drive sprocket at the tip for damage. Most people just need to loosen the bar nuts and adjust.
