What cooler accessories do I actually need?

You really only need two: a removable divider to keep drinks separate from food, and a cutting board that fits inside. Everything else — bottle openers, baskets, cup holders, fancy drain plugs — is either built into the cooler already or a waste of space. A decent divider stops soggy sandwiches, and a thin plastic cutting board doubles as a shelf or prep surface.
