How can I tell if an extension cord is overheating and a fire risk?

A hot or warm cord or plug that’s uncomfortable to hold for more than a few seconds is the main sign. Other clues include a burning smell, visible melting or discoloration near the plug, a stiff or brittle cord, a faint buzzing sound, or flickering lights. Unplug it immediately and replace the cord — extension cords are cheap, but house fires are not.
